The patient forms for new patients have to be filled out 24 hours prior to your scheduled appointment time. If the forms are not filled out within this timeframe there is a possibility that your appointment will be cancelled. This is a part of the office’s policy because it gives the provider enough time review your paperwork before your scheduled appointment.
